Knox County School employees may elect to participate in a supplemental insurance plan. The employee will choose a vendor and contact them directly. The employee must then complete the Payroll Deduction/Change Authorization form.
All forms must be completed and given to the vendor agent or the Benefits office. Forms are due by the 5th day of the month September – June for employees paid monthly. No changes will be made during the months of July and August. When an employee wishes to make changes in the amount being withheld, the employee must submit a new form with the corrected amount.
