Proximity Cards (aka Prox Cards) are the KCS badges that allow access into the buildings. If you are requesting an employee's first Prox Card, need to replace an existing one, or need to update security access, please follow the steps below.
LOGGING IN AND STARTING A REQUEST
- When opening Operations Hero, choose the Sign in with Microsoft option.

- On the Operations Hero dashboard, click the New Request button at the top of the screen. A New Request pop-up window will open.

COMPLETING THE NEW REQUEST POP-UP WINDOW
Below are the various fields to be completed before submitting the request. There is a screenshot below the descriptions to show the location of each field.

WORKFLOW: select SPRX: Prox Card Request.

REQUESTER: make sure it has your name.

CATEGORY: select KL:Card Card Access.

ASSIGN TO: do not enter anything in this field as it is for internal Maintenance use.

LOCATION: select your location. (Note that some users may have multiple locations from which to choose.)

DESCRIPTION/SUMMARY: provide additional information that may not be covered in the other steps.

NAME OF PERSON NEEDING BADGE: Enter the “go by” and last name of the person as you want it to appear on the card. (For example, Christopher may go by Chris, or the person may go by his/her middle name instead of first name.) Do not use nicknames.

EMPLOYEE ID#: enter the person's KCS ID number.

REQUEST ACCESS TYPE: select the type of access for the card. Choices are:
- Admin
- Child Care
- Faculty
- Custodial
- Summer
- Kitchen
- Subs

Med Box

REASON FOR NEEDING NEW BADGE: select the reason for requesting the card. Choices are:
- Lost (this includes stolen cards as well). For Lost/Stolen cards, see Step 21.
- Quit Working
- Damaged
- New Hire

Other

GIVEBACKS CONFIRMATION NUMBER: If the card was lost or stolen, the replacement will need to be purchased through Givebacks prior to completing this request. The KCS Help tutorial “Ordering Replacement Prox Cards and Keys in Givebacks” explains the process. Once the order is complete, he/she should provide you the Givebacks confirmation #, which should be entered in this field. (If the card was not lost or stolen, ignore this field.)

PHOTO: select YES showing that you have confirmed that the photo being uploaded only contains a head and shoulders image (not waist up, full body, or any other perspective).

UPLOAD ATTACHMENTS: drag and drop the image or choose Select Files to navigate to file to upload the image.

SAVE REQUEST: click the button to submit your request.
